This questionnaire is used to determine your needs and desires for your logo design concepts. All designs are based on the information that you provide in this questionnaire. Be very specific and give as much information as possible. The future of your brand is at stake. If I need clarification or additional information, I may ask to schedule a phone consultation with you as well.

*Describe your business to me. I don’t need to know what you do but what values are important to you in your business. Tell me your goals and dreams and what you want people to know about your business
 
*Describe your personal style to me. How do you dress and what style is your home? What inspires you?
 
*What do you want your logo to say about your business?
 

*What style of logo do you want? (clean, sweet, elegant, etc.?)

 

*What colors represent your business? Are these colors important for you to keep (do you love them???). If you don’t have colors or want to change them, do you have any in mind?

 
*Are there any elements you want to include in your logo/brand? For example, do you see your logo including your initials, or maybe a circle, butterfly or flower? How important is it to you that it be included? Or is it important to you that there are NO elements included other than your business name?
 
*Any preferences on type styles (script, handwritten, etc?)
 

*Please show me AT LEAST 3 logos that you like and 3 that you don’t like (photographers are ok but I would PREFER non-photographers logos) Include links or images. Can you tell me what you like about them or don’t? The more you can show me, the better!
 

*If you currently have a logo, what do you like about it? What don’t you like and why do you want a new/updated logo? Please show me your current logo.
 

*It would be very helpful for me to see anything that you plan to keep or start using your logo along with (web site, business card, brochures, etc.) Please email them to eva@evatalley.com if you have them.

 
 
 
  Terms and Policies  
 
  You must read these terms and policies and agree to them in order to proceed with this agreement. You are consenting that you have read and understand all terms and policies and that you have completed this contract fully and truthfully. I. Payment and Refunds
Full payment is due up front before any work will begin unless otherwise specified. Due to the custom nature of these products/services, no refunds will be awarded once work has begun.

II. Turnaround Times
Please be aware that I often have a waiting list – I will discuss this with you in advance. Remember that design is a creative process and turnaround times can often be hard to predict. Turnaround also largely depends on how quickly you respond. Turnaround can be 3 days, 3 weeks or even 3 months and depends on the project and the ease of communication. I try to complete all projects as soon as I can, but I cannot guarantee a specific timeframe. Please be patient and check in with me if you have any questions or concerns.

III. Cancellation
If for some reason you need to cancel your order before work has begun you must submit a request in writing (via email). I will take your request into consideration and let you know if a refund is possible.

IV. Copyright
Under US Copyright law, the designer / artist is the owner of all files and artwork created for the client. All designs are registered formally with the US Copyright Office.

V. Client Usage
Designs may be used for print, web, television or other media. Custom Logos cannot be altered in any way without written permission. Pre-Approved alterations include color changes, tag-line removal and the stand-alone usage of the graphic logo-mark portion of the design (such as illustration or initials). Please do not alter the layout, scale or overall feel of the design without written permission.

VI. Final Files
Clients are allowed to choose only one final logo design. Final designs will be delivered electronically and if requested via CD by mail as well. You will receive the original vector file as well as a layered Photoshop file. You may also request other files, such as but not limited to, png, jpeg or pdf at no additional cost.

VII. Miscellaneous
All designs, concepts, slogans or other materials displayed by the designer, which are not selected will remain the full property of Eva Talley / Eva Talley Creative. All designs / artwork remain copyrighted by Eva Talley / Eva Talley Creative. Prices and Policies are subject to change without notice.
